MUST be complete and returned to ANHS Grad Nite
for your registration to be valid!
Failure to complete the registration form IN IT'S ENTIRETY will void your Grad Nite ticket!
Please follow these instructions:
Download and print
the file below, which is your registration form. If you already have a
registration form, you may skip this step.
Complete the form,
PRINTING CLEARLY. Please include ALL information! Both the student AND
parent must sign the form, acknowledging the rules established for Grad
Nite.
Make your Grad Nite payment, by check (until June 1, 2011) or PayPal.
If using PayPal:
Please put STUDENT NAME in the comment/note section of the PayPal transaction.
Attach a copy of your PayPal receipt to your registration form.
Return the completed
form AND payment (your check or copy of PayPal receipt) to Grad Nite.
Completed forms may be turned in at the Activities Window at ANHS or
mailed to:
